At my McHenry physical therapy clinic, one of the most common causes of heel pain I treat is plantar fasciitis. With the right physical therapy approach, plantar fasciitis is highly treatable.

What Is Plantar Fasciitis?

Plantar fasciitis is an overuse condition involving inflammation of the plantar fascia, a thick band of tissue that runs across the bottom of your foot and connects your heel bone to your toes. It acts like a shock absorber and supports your arch. When this tissue becomes irritated or inflamed, it causes sharp or stabbing heel pain, especially during your first steps in the morning or after long periods of rest.

How Do You Get Plantar Fasciitis?

Plantar fasciitis develops from repetitive stress to the foot, which can lead to microtears in the fascia. Contributing factors include:

  • Sudden increases in activity or mileage
  • Long periods of standing or walking on hard surfaces
  • Flat feet or high arches
  • Poor footwear with inadequate support
  • Tight calf muscles or Achilles tendons
  • Obesity, which increases strain on the fascia

This condition is especially common among runners, service workers, teachers, and anyone who’s constantly on their feet.

Symptoms of Plantar Fasciitis

The hallmark symptom is heel pain, often described as:

  • A sharp or stabbing pain in the bottom of the heel
  • Worse in the morning or after sitting for long periods
  • Eases after moving around, but may return after long activity
  • Tenderness to touch near the heel bone

In chronic cases, people may alter how they walk, which can lead to pain in the knees, hips, or lower back.

Treatment Options for Plantar Fasciitis

The earlier you start treating plantar fasciitis, the better. Most cases can be resolved with conservative care, including:

  • Physical Therapy – Customized exercises to stretch, strengthen, and restore balance in your foot mechanics
  • Footwear Modifications – Supportive shoes or orthotic inserts to offload the fascia
  • Home Remedies – Ice massage, rest, and elevation
  • Anti-inflammatory Medications – As directed by your physician
  • Night Splints – To keep the plantar fascia gently stretched overnight
  • Manual Therapy – Skilled hands-on techniques to release tight muscles and fascia (a specialty in our clinic)

In rare, persistent cases, steroid injections or even surgery may be considered, but I find that dedicated physical therapy resolves symptoms for the vast majority of my patients.

Physical Therapy Exercises for Plantar Fasciitis

When you work with our experienced physical therapists, your treatment plan will include evidence-based exercises targeting plantar fasciitis’s root causes. Here are a few examples of what we might incorporate in your physical therapy sessions:

1. Calf Stretches

Tight calves increase strain on the fascia. Regular stretching can reduce tension and prevent reinjury.

2. Plantar Fascia Stretch

Sitting down, cross your affected foot over the opposite knee and gently pull your toes back until you feel a stretch in your arch.

3. Towel Curls

Place a towel on the floor and use your toes to scrunch it toward you. This helps strengthen foot muscles and support the arch.

4. Marble Pickups

Pick up marbles with your toes and place them in a cup. This improves foot control and coordination.

5. Heel Raises

Strengthens the calf and supports proper foot mechanics. Start with both feet and progress to single-leg raises as tolerated.

6. Ice Bottle Roll

Roll a frozen water bottle under your foot for 10–15 minutes. This reduces pain and inflammation.
